ACCESSING YOUR ONLINE MEDICAL RECORD Spooner Health System provides patients a way to find their medical information online by using something called a Patient Portal. If you have been an inpatient or observation patient with Spooner Health System, you can view the following information online after you leave the hospital: Allergies Immunizations Medications that you take and medications that you no longer take. Health problems Test results You can look at information from your hospital stays and from same-day tests and procedure results only. Information from visits to the Emergency Department, Outpatient Surgery, Home Care, Radiology, Rehabilitation Services, Nutrition Education and Cardio-Pulmonary will not be available online. Page 1 of 6 REGISTERING YOUR ACCOUNT These instructions will guide you through the registration process. It is important to follow these instructions in the order they are given. Step 1: Confirmation Once you leave the hospital, an will be sent to the address you provided during registration. Check your inbox or spam/junk folder for the message to begin your registration process. The will be sent from and the subject line will state Spooner Health System Patient Portal New User. Open the and click the link provided. You will go to a new webpage: http://www.mymedicalencounters.com where you will be asked to register as a new user. Step 2: Registration This website is secure. Spooner Health System may not use or give out protected health information in a manner inconsistent with its Notice of Privacy Practices. You will be asked to enter your name, date of birth, your own user ID and password (8 characters long consisting of at least 3 different types: capital letters, lower case letters, numbers, and special characters such etc). Very important: Make sure you check the box at the right to verify that your information is correct. Then click the REGISTER button at the bottom of the screen. (See Figure 1) Figure 2 Figure 1 Page 2 of 6 Step 3: Security Question Choose three security questions, fill in the answers, and then click the Submit button at the bottom of the screen. (See Figure 2) Figure 2 Page 3 of 6 Step 4: Patient Record From the patient list, click on the name of the patient you are authorized to view. The patient s visits will be listed with the most recent visit on top. Next to each visit there is a link that shows the Visit Number and the title View Summary of Care. Click on the link in the View Summary of Care column to view the document. (See Figure 3) Step 5: Logout Figure 3 Please click Logout (top right side of screen) when you are finished reviewing your medical records. This ensures the connection is closed and there is no chance for inappropriate access to your portal. Page 4 of 6 ACCESSING YOUR ACCOUNT Log On: Once you have completed the registration process, you can access your medical information at any time, from any computer with internet access. To do this, go to the patient portal webpage at http://www.mymedicalencounters.com. Enter your User ID and Password (See Figure 4). Click on Login button. Figure 4 Page 5 of 6 Questions? If you did not receive the confirmation , please call between the hours of 7:00 am and 9:00 pm and ask for a portal reset for another to be sent to you. FREQUENTLY ASKED QUESTIONS Is there a way to get in to my accounts from both work and home? Yes, you can get to your accounts from any computer that has an Internet connection and a browser that supports secure communications (Internet Explorer, Mozilla Firefox, Google Chrome or Safari). Can anyone see my information? Only you and anyone you designate as your Authorized Representative will have access to your information. Please be aware that anyone you provide your user ID and password to will also be able to view your information. Do I need extra software or hardware to use clinical information? No, extra software is not needed. You can access information from any computer that has an internet connection and a secure communications enabled browser (Internet Explorer, Mozilla Firefox, Google Chrome, or Safari). How do I register for an account? Access the web link from the invitation sent to you at the address that you provided during registration or answer the question on the new registration screens to create your user ID and password. How do I view the Summary of Care? Click on the View Summary of Care or View Transfer/Referral of Care link for the desired visit. How does clinical information work? Clinical information is a collection of all your visits allowing you to view services and procedures for any given service date. Page 6 of 6
